The first day of FALL Sports is August 12, 2013.

PIAA physicals need to be completed after June 1, 2013 and turned into the Main Office or to each player's coach for the first day of practices.

The date for $10.00 physicals to done at school is July 12th. Parents of students or sudents wishing to have one completed at school need to call the Athletic Office, 434-2501, Ext 517, prior to July 7th to set up an appointment.

In the upper right hand corner of the page enter "Mountain View Jr. / Sr. High School, Kingsley, PA." This will take you to the main athletic page for the high school. You can then click on "schedules" then select the month. You will now be given up to the minute information pertaining to all athletic contests for Mountain View Jr. Sr. High School for the month that you selected..  


PIAA Physical Form Information

The following information pertains to the PIAA physical (CIPPE) forms that are required to be turned in by all student athletes every school year and prior to each athletic season.

  • All students participating in athletics must have a physical examination each school year before practicing or participating in athletics.
  • Physicals are scheduled on July 12, 2013, for the following school year.
  • You can pick up the pick up the PIAA Physical (CIPPE) Forms in the Main Office.
  • The cost of the physical will be $10.00.
  • Sections 1-5 and the 1st part of section 6 of the physical form need to be completed by the parent and returned to the Athletic Director. On the "Sports" line, write all sports the student is interested in playing for the year.
  • The Doctor completing the physical examination will complete the bottom portion of section 6 of the form.
  • Once the Physician clears the student for participation, the physical examination is good for the entire school year.
  • Students that participate in more than one sport must complete section 7 for each additional sport.
  • There are questions on section 7 that pertain to any injury that may have occurred during the previous athletic season. 
  • If all questions on section 7 are answered "No", the form needs to be signed by the athlete and parent and returned to the Athletic Director. The athlete is then cleared for the next athletic season.
  • If any questions on section 7 are answered "Yes", the student athlete needs to have a re-certification physical completed by the family doctor. The re-certification physical will determine whether the athlete is cleared to play the next athletic season.
